Children's Health Homes In New York

Children in NYS who are currently on Medicaid, between the ages of 0-21, and meet criteria for one of the Health Home Target Populations are eligible to enroll in a Health Home Care Management agency.

The target populations include:

1) Having a Serious Emotional Disturbance (Bio-polar, Depression, Schizophrenia, Substance Abuse Disorder, Eating Disorders, and several other DSM diagnoses)

2) Having 2 chronic medical conditions (Use this link to view the full list of approved chronic conditions)

3) Sickle Cell Anemia

4) HIV

5) Complex Trauma
